SCAN Silver is a Medicare Advantage Health Maintenance Organization (HMO), offering all the benefits of Original Medicare plus more! SCAN Silver Health plan does not include the NEW Part D prescription drugs coverage. For more information on the Part D prescription drug coverage please contact Member Services at 800.320.2688.
SCAN Silver health plan has a $0 monthly premium! You also continue to pay the Medicare Part B premium of $96.40 each month. You can visit any doctor in our network and choose among our network hospitals, including St.Joseph's, Dameron and Lodi Memorial.

Anyone with Medicare may apply for SCAN Silver, including the disabled. Applicants must live in San Joaquin County.
