SCAN contracts with Express Scripts, Inc. to provide Part D Prescription Drug Program benefits to all members of SCAN Gold and SCAN Gold-Select. Express Scripts has over 50,000 pharmacies throughout the United States to serve you. You may also choose to use Express Scripts' mail order service, which is a convenient and cost saving method of obtaining medications that you take on a routine basis. As a member of SCAN Gold or SCAN Gold-Select you have access to Express Scripts Call Center 24 hours a day.

In order to properly process pharmacy claims the following information will be necessary. This information is present on the member's Application Acknowledgement Letter, their Enrollment Acceptance Letter, and the Member's Pharmacy Identification Card. It is repeated here for convenience.
